Farmington, CT  – Residents of New Horizons Village, a community providing accessible living for adults with physical disabilities, have received a new van thanks in large part to the contribution made by Farmington Bank Community Foundation.

New Horizons operates seven vans daily; each van averages 1,524 miles monthly, transporting residents for all their daily non-medical travel needs i.e. groceries, religious services, school, work, volunteer work, shopping, family functions etc. Half the fleet of vans has in excess of 200,000 miles.

 

For transportation to medical appointments, residents contact Logisticare, a centralized state vendor, which coordinates transportation to and from scheduled, non-emergency medical appointments.  Medicaid reimburses for this type of non-emergency medical transportation.  However, there have been a number of glitches in the system that have resulted in missed medical appointments, delays in medical care and follow-up. 

New Horizons, in an effort to ensure that residents get to their medical appointments in a timely manner, has purchased a new ADA approved, side loading van, paid in part with the grant from Farmington Bank Community Foundation. The addition of this van will allow New Horizons to assist with non-emergency medical transportation for residents, particularly those that have critical medical appointments.  This added resource will benefit both New Horizons Village residents as well as the residents of Cherry Brook Health Care Center.  New Horizons, Inc. is the owner of Cherry Brook Health Care Center, a not for profit skilled nursing facility providing both short term rehabilitation and long term care located in Canton.
